The Certification Process

Acquiring a medical cannabis card in Illinois does not get any easier than this. When you come to our clinics, you will be treated with the attention and respect you deserve. From the moment you book an appointment with us, our team will guide you every step of the way through the process until you have your card.

Patients also come to us for cannabis guidance after they receive their card.

Qualifying Patients

In order to get a medical cannabis recommendation and legally purchase cannabis products from dispensaries some basic qualifications must be met.

  • Must be 18 years or older  (minors need two physician recommendations as well as consent from their legal guardian),
  • Illinois state resident with proof of residency
  • Diagnosis of a qualifying medical condition
  • Have certification from a physician

For parents or caregivers attempting to access medical cannabis for their children, two physician certifications are required, along with the parent or legal guardian serving as primary caregiver for the patient.

Step 1: Schedule An Appointment

Schedule an appointment with us at 888-372-0325 or schedule online to start your evaluation process! We will answer any questions you have about the evaluation process. We will also explain the need for your medical records documenting a qualifying condition and if necessary, obtain the records for you after you sign a medical release form. Learn more about the benefits of obtaining a card here.

Step 2: Physician Consultation And Certification

When you arrive for your appointment, our staff will provide you with our intake forms. Once completed, they then introduce you to the doctor. The physician will review your records, perform an exam, discuss your condition and history, and answer any questions you have regarding medical cannabis. If you have a qualifying condition the doctor will approve your certification.

Step 3: Provisional Access

The morning after your application has been submitted, the Illinois Department of Public Health (IDPH) will then upload your provisional access letter, which we will download and email directly to you. Once received, this letter along with your ID will allow you to purchase medical cannabis at the licensed Illinois dispensary you selected during the application process. The provisional access letter is valid for six (6) months and should be used until your medical cannabis card arrives in your online patient portal. You may visit any medical dispensary at any time. Medical cards are not honored at recreational dispensaries.

Step 4: Get Your Medical Cannabis Card

Upon processing and approval of your submitted application, the state Department of Health (DOH) will then send out your Medical Cannabis card, which you will receive by your patient portal where you will be able to print it out. Once received, this will allow you to continue purchase medical cannabis at any medically licensed Illinois dispensary.
